Hospice Medical Director

Hospice Medical Director oversees the medical care provided to patients, ensuring compliance with hospice policies and regulations.

Hospice Administrator

Hospice Administrator they can range from managing community health services, overseeing clinical support services, coordinating medical programs and managing budgets.

Hospice Nurse

Hospice Nurse coordinates patient care and provides direct nursing care, assessing patient needs and developing individualized care plans.

Certified Nursing Assistants (CNAs)

Certified Nursing Assistants (CNAs) provide personal care to patients, such as bathing, dressing, and feeding, assist with mobility and exercises, and monitor and report changes in patient condition to the nursing staff.

Chaplain

Chaplain provides spiritual support and counseling, assesses spiritual needs, facilitates communication with the patient's faith community, leads or participates in religious services and rituals, and offers grief support for families and staff.

Bereavement Coordinator

Bereavement Coordinator provide grief counseling and support to families before and after the patient's death; developing and implementing bereavement care plans, offering individual, group, and family counseling sessions, facilitating support groups and workshops on grief and loss, and coordinating community resources and referrals.

Medical Social Worker (MSW)

Medical Social Worker (MSW) assesses the psychosocial needs of patients and families, provides counseling and emotional support, assists with advance care planning and end-of-life decisions, helps patients and families navigate healthcare and community resources, and advocates for patient and family needs within the healthcare system.

Wound Nurse

Wound Nurse assesses and treats wounds, including pressure ulcers and surgical wounds, develops and implements wound care plans, provides education on wound prevention and care, monitors the healing process, and collaborates with the hospice team to ensure comprehensive care.

Ostomy Nurse

Ostomy Nurse provides care and education for ostomy patients, assesses stoma and peristomal skin, develops individualized ostomy care plans, educates patients and caregivers on ostomy management and self-care, and offers support and resources for adjusting to an ostomy.

Hospice Volunteers

Hospice Volunteers provide companionship and emotional support, assist with light household tasks and errands, offer respite care to give primary caregivers a break, participate in activities and events organized by the hospice program, and help with administrative tasks and fundraising efforts.
